Responsible for the operations and leadership of the inpatient and emergency department care management program for CMC. Maintains overall fiscal responsibility for the department and performs duties to support the goals and objectives of the Carilion Clinic system. Responsibilities include: Outcomes Management, Utilization Management, Reporting/Data Analysis and Staff Management. Effectively represents Carilion Clinic within the healthcare community and to the public.

Responsible for alignment of all strategic case management initiatives across the system.

Organizes and develops strategies and processes to support the transition of patients into the post acute environment.

Responsible for the alilgnment of case management practices across the continuum of care by developing relationships with other case management leaders in the system.

Partners with key stakeholders (including medicine, nursing, outpatient case management and post-acute care) to develop and execute operational plans for managing care transitions and transition outcomes. Manages multiple, complex and potentially large departments, as well as designated administrative and support staff to assure cost effectiveness and achievement of goals and budgets. Monitors performance quality and performance metrics while also providing recommendations on operational design and efficiencies.

Will support and guide system-wide care management functions. Demonstrates a high level of change management, emotional intelligence, and leading with influence skills. In addition, this role will support and manage relevant project implementation activities, change project activities and implementation best practices, policies and processes throughout the entire organization.

Recognizes and acts upon opportunities for growth and improvement to advance goals of the organization. Utilizes project management principles to accomplish goals, seeks out appropriate resources, and proactively anticipates changes within the market.

Demonstrates a high level of proficiency in leadership development of the departmental leadership. Facilitates professional growth and development including orientation in service programs and effective performance improvement systems. Communicates, organizes, prioritizes, leads, facilitates, coaches and problem solves with reporting leadership staff to ensure that goals, objectives, and budgets are met.

Minimum Qualifications

Education: Masters Degree Required: BSN required.

Experience: 5-7 years experience in management demonstrating increased and/or diversified responsibility in a system wide framework.

License: Must be licensed as a Registered Nurse in the State of Virginia or have a multi-state compact RN license. Certified Case Manager strongly preferred.

Other Minimum Qualifications: Advanced leadership, planning and organizational skills to manage multiple and potentially large, complex departments and to direct the successful completion of initiatives and achievement of fiscal goals. Demonstrated interpersonal and communication abilities to support and promote a respectful, inclusive culture of service excellence. Proven skills in clinical, operational and human resource management, including staffing, planning, performance management, team work and process improvement. Statistical analysis and fiscal management skills.
